Baldwin Care is a family owned retirement living company with villages in NSW, QLD and TAS. We are now licensed to provide Home Care Packages under the new Consumer Directed Care (CDC) regime. CDC allows consumers to choose their own provider and to direct the funding to that provider.

We offer our residents a range of support services including personal care, help with laundry or shopping and basic maintenance to enable them to stay in our villages for as long as possible. This newly created role will be responsible for developing, implementing and reviewing care plans and services for the residents within our villages. The services will then ideally be delivered by our own Baldwin Care staff or by local providers that we partner with ensuring that program requirements and the consumer directed care model are met. The role requires some travel both around Sydney and interstate. A current drivers-license is a must.

Key responsibilities include:

  • Educating and informing our residents about CDC and the services Baldwin Care can provide
  • Coordinating and implementing care plans that are safe, timely, effective, efficient, equitable, and client-centered
  • Handling case assignments, drafting Home Care Package plans, reviewing case progress and determining case closure
  • Helping residents achieve wellness and independence and making informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Taking the extra mile and interacting with residents to keep track of their progress and to ensure satisfaction
  • Recording case information, completing all necessary forms and producing reports where required

About you:

  • Certificate III or Certificate IV in Aged Care or Community Care or related tertiary degree
  • Case management experience in Aged Care
  • Preferably knowledge of CDC
  • Excellent written and verbal communication skills
  • The ability to build and maintain rapport with people quickly and easily
  • The ability to work independently and as part of a team
  • Excellent time management, attention to detail and record keeping skills
  • A current driver's license
  • Strong computer skills (Word, Excel and Outlook)
