Baldwin Living HomeServe

Baldwin Living HomeServe aims to provide supported living environment within your home settings and promote wellness and independence of all residents.

With the introduction of Consumer Directed Care (CDC) by the Federal Government, the decision about how funding is spent for care at home has been given to the individual. Under CDC, people will have more choice on how their care is delivered and their funding is allocated.

Baldwin Living HomeServe, being an approved provider, is set up to provide you assistance in understanding the range of choices available and to help you in making informed decisions about the kind of care you can receive and how funding is spent.

WHAT IS CONSUMER DIRECTED CARE?

Consumer Directed Care (CDC) is a model of service delivery designed to give more choice and flexibility to consumers. Consumers who receive a Home Care Package will have more control over the types of care and services they access and the delivery of those services, including who delivers the services and when.

From 1 July 2015, all Home Care Packages are delivered in a CDC basis.

The program aims to provide long-term support where consumers get to choose the services and support that will make a difference to their life.

There are four levels of Home Care Packages for different levels of care and support needs:

  • Level 1 for basic care needs
  • Level 2 for low-level care needs
  • Level 3 for intermediate care needs
  • Level 4 for high-level care needs

HOW IT WORKS

Home Care Package funds aren’t paid directly to you. Instead, you choose an approved Home Care Provider, in this instance Baldwin Care Services, to administer the funds on your behalf and give you ‘case management’ support and service.
